2012 Penalty Renewal FormThe 2012 Penalty Renewal Form is now available. Please click here to download the form. Effective January 12, 2012 - ALL applications, including supporting documentation, and letters of request to re-take the PE Exam are required to be in the Board office no later than thirty (30) days prior to the next scheduled Committee meeting.
Applications and PE exam re-take requests received after the thirty (30) day deadline will be processed and presented at the following Committee meeting.
